INDIVIDUALIZED TREATMENT PLANS

Few of The UP Foundation’s participants receive all available services. Rather, counselors and Certified Peer Recovery Specialists (CPRS) collaborate with participants to create Individualized Treatment Plans (ITP). In this way, participants have the opportunity to assume an active role in their own recovery. Each ITP is unique to the participant it is created for, consisting of one or more of the following services:

          • CPRS are available 24/7 to assist participants with all recovery related problems, and to assist during crises.

        • Peer Support Meetings

          • Peer Leader run groups

        • Transport to and from 12-Step Meetings

        • In-home services

          • Family and peer support provided by CPRS
